Use this form to request Office 365 student-employee accounts/mailboxesand departmental/generic mailboxes.

Your request will be processed through the Web Help Desk ticketing system and will allow you to track its progress.

IMPORTANT: New employee mailboxes are now created automatically as soon as the employee's Active Directory account has been created.


Email Address Format
Employees -   first.last#@montana.edu
Student Employees - firstlast#@montana.edu  (no dot between first and last)

Student Employee Account Login Info:
These credentials will be used to log into work computers or resources.
User name: firstlast#  (no dots or spaces)
Password: NetID password

Before you begin please refer to the required info for the mailbox type you're requesting so that you will have the right information to complete this form.

Mailbox User Information
Enter the contact information of the student for whom this mailbox is being created.
Requester Contact Information
Please enter the contact information for the person requesting the mailbox/account (usually the student's supervisor).

Indicate how the departmental mailbox will be used:
Primary Mailbox Owner

The primary owner is responsible for the use of the mailbox and is able to request delegate access for other users.

Enter email address (@montana.edu) of the person who will be the primary mailbox owner.
Delegate Users 

Enter the address for each person who should have access to this mailbox and choose the type of access he/she should have.

Mailbox Access Types
  • Send as - Can send emails as the address, can't view other emails in the mailbox.
  • Send on behalf - Can send emails on behalf of the address, can't view other emails in the mailbox.
  • Read and manage - Can read and edit but not send from the mailbox
  • Full access - Can send as read and edit emails.
